Patient Information: Tricyclics for Pain & Sleep
Frequently Asked Questions
Why have I been prescribed a Tricyclic?
Tricyclics are a family of medications that include Amitriptyline, Nortriptyline, Imipramine, Dosulepin, and Lofepramine. While originally developed as antidepressants, they are highly effective for treating nerve (neuropathic) pain.
Standard painkillers often fail to target nerve-related discomfort. Tricyclics work by altering how your brain and spinal cord process pain signals.
The "Vicious Circle" of Pain and Sleep
When pain persists, it disrupts sleep. Waking up unrefreshed makes it harder to manage pain the next day, creating a cycle that is difficult to break.
- Our Goal:Reduce pain, re-establish a healthy sleep pattern, and break the cycle of chronic discomfort.
Side Effects & Considerations
While many people tolerate tricyclics well at low doses, they can cause side effects. Most of these appear early and often diminish after the first week.
Common Side Effects
- Dryness:Dry mouth, dry eyes, or blurred vision.
- Drowsiness:This is why we recommend taking the dose at night.
- Gastrointestinal (GI) Issues:Constipation is common. Some patients may also experience nausea or changes in appetite.
Less Common but Important
- Urinary Issues:You may find it difficult to start urinating or feel that your bladder is not emptying completely (urinary retention).
- Glaucoma Warning:In rare cases, tricyclics can increase pressure in the eye. If you have a history of glaucoma, please inform your doctor before starting this medication.
- Heart Rhythm:At higher doses, these can occasionally affect heart rate.
Drug Interactions & Safety
Tricyclics can interact with other substances. Always inform your doctor or pharmacist about all medications you are taking.
| Interaction Type | Guidance |
|---|---|
| Alcohol | Alcohol significantly increases the sedative effect (drowsiness) of tricyclics. It is best to avoid alcohol, especially when starting treatment. |
| Painkillers | It is generally safe to take these with standard painkillers (like paracetamol), but avoid other strong sedatives unless specifically prescribed. |
| Other Antidepressants | Taking tricyclics with other antidepressants (like SSRIs or MAOIs) can lead to serious reactions. Ensure your specialist is aware of your full medication list. |
| Herbal Remedies | Avoid "St. John's Wort" as it can interact poorly with tricyclics. |
How to Take Your Medication
- When:Take your dose as a single tablet one hour before bed.
- Morning Grogginess:If you feel “hungover” in the morning, try taking the dose earlier in the evening (e.g., 7:00 PM or 8:00 PM).
- Missing a Dose:If you forget a dose, skip it and continue as normal the next day. Never double your dose.
- Driving:If you feel drowsy or your vision is blurred, do not drive until the effects have completely worn off.
